University facilities may be utilized for certain events, including meetings, seminars, luncheons, performances, and banquets, but are predominantly for use by the University’s departments and sponsored organizations. Additional leasing is available for non-profit organizations ONLY. Facilities scheduling is coordinated through the department of Procurement and Contracts. Please review the information below and contact 386-754-4226 or [email protected] if you have any questions.
A request form must be completed and approved. Download form here:
Facilities Use Request, Updated April 2022
*Note: Download document before filling or information may be lost. Fields do not work in some browsers. Currently, digital signatures are not authorized. Please print, hand-sign, and either mail to our office or scan and email.
When making a request, please be sure to thoroughly read the entire document and complete pages 1-4 only. Fees vary according to the type of user and requirements. Once the paperwork is completed it should be returned to the Procurement & Contracts Department
for further distribution. This must be done at least 30 days prior
to the event. It is important for paperwork to be returned in a timely manner so that arrangements can be made for the scheduling of services (i.e. Custodial and Maintenance for facility set-up and cleaning, Audiovisual for public address systems, Public Information for event promotion, and Security for various needs.) In addition to the contract, we must have your organization’s certificate of liability insurance, W9 if you have not done prior business with the University, and proof of tax exemption.
The forms may be emailed to [email protected] or mailed to:

*Note: Only upon receipt of the fully executed contract, certificate of insurance, and payment of all fees, will the facility be reserved.
Do not rely solely on the reservation approval to confirm your needs. A follow-up phone call to the following service coordinators is helpful to prevent problems due to possible lost paperwork or errors in paperwork.
